Incentive Claim Form

This form should be submitted once per week. You can submit up to 3 gift cards in a 7-day period beginning on Sunday and ending on Saturday. We have removed the 48 hour limitation on submissions.

We have also enabled two recording models.

Block Submission: You may submit time as a single 5-hour block.

Non-Block Submission: You may submit up to 5 increments of time adding up to 5 hours. These must be a minimum of 1 hour. For example, if a member spends 3 hours on Monday, and 2 hours on Tuesday, they could record this and receive 1 gift card.
