The below form was created for any member who wishes to file a complaint with the Disciplinary Action Committee regarding another member. To initiate the process, complete the form below by entering your information as the complainant, the information about the person you are accusing (respondent) and information about the event.

If you have evidence, you can select what kind you have. The DAC will contact you to obtain any photos, videos, or screenshots. If you select “Incident was caught on security cameras” the Security Committee will conduct a review and submit the information to the DAC.

Complete the form by signing. Be aware that by signing you are initiating a DAC complaint against the member named under Respondent and any additional members you name. Intentionally making false reports is a violation of company policy and will be dealt with.
